Immunizations

Required Immunizations For All Students:
DTP (Diptheria, Tetanus, Pertussis): 3 dates
Tdap: 1 date within the last 5 years
Polio: 3 dates
Measles: 2 dates
Mumps: 2 dates
Rubella: 1 date
(2 MMR's meet the Measles, Mumps, Rubella requirement)


Additional Immunizations required:
*If you are an international student
TB Skin test (PPD): 1 date within the last year

*If you are applying to the nursing program
Varicella Titer
Rubella Titer
Hepatitis B: 3 dates
TB skin test (PPD): 1 date within the last year

*If you are an Athletic Training Student
Hepatitis B: 3 dates
TB skin test (PPD): 1 date within the last year
