Required Forms for Athletic Participation
Student Athletes
Please complete fully and return all pages of the designated forms below. The medical forms should be MAILED to the address noted below by August 5 for early-returning, fall-sport athletes and by August 17 for all other athletes.

Reminders
- All student athletes MUST send a photocopy of your personal insurance card (front and back). Mail with all forms you return.
- All NEW/TRANSFER student/athletes MUST provide a physical examination prior to participating in any activity with their team. The medical exam required for admittance to school will be acceptable. Make a copy of this physical and send with the other athletic forms. Mail all student health forms back to student health.
- All medical forms must be completed in BLACK or BLUE ink only.
- Medical forms CANNOT be accepted by email or fax.
- Mail the completed athletic medical forms to the address above ? DO NOT include them with other campus required mailings (such as financial aid forms, housing forms, physical/medical forms for student health, etc).
- Please make copies of all your completed forms in case the originals get lost in the mail.
- ALL student athletes must read the Barton College policies on Drug Education and Testing, Alcohol, Tobacco, Banned Substances, and Performance Enhancing Drugs.
- All parents/guardians and student athletes should read the information pertaining to the Athletic Insurance Coverage Information.
- ALL FORMS MUST BE COMPLETED IN FULL AND RETURNED BEFORE THE ATHLETES WILL BE ALLOWED TO BEGIN PRACTICE WITH THEIR TEAM. Failure to submit the forms by the deadline will most likely delay your ability to begin practice with your team. The physical MUST be included for ALL NEW/TRANSFER STUDENTS! Do not bring the forms with you the first day of practice or school. You will not be allowed to practice that day.
